Candidate Profile

The ideal candidate for the Practice Transformation Facilitator role will possess the following qualifications:

  • Educational Background: A Bachelor's degree or equivalent, demonstrating a strong academic track record.
  • Professional Experience: A minimum of 3 years in clinical practice, with preference given to candidates having over 5 years of relevant experience in the healthcare sector. Experience in primary care settings and successful quality improvement initiatives is highly desirable.
  • Innovative Mindset: CINQCARE seeks to address longstanding gaps in care delivery for Black and Brown populations. This role requires a proactive approach to ensure CINQCARE effectively fulfills its mission, showcasing urgency and results-driven performance in a dynamic healthcare landscape.
  • Communication Skills: Exceptional verbal and written communication abilities, with a talent for presenting complex concepts clearly to diverse audiences, including team members, investors, and community partners. Proficiency in Microsoft Office applications is essential.
  • Relationship Management: Proven ability to cultivate and maintain productive relationships with business leaders and external stakeholders.
  • Organizational Culture: Strong ethical standards, good judgment, and a collaborative spirit; a commitment to personal and professional growth within a fast-paced, entrepreneurial environment.

Role Responsibilities

The Practice Transformation Facilitator will report to the Practice Transformation Manager and will be responsible for providing strategic insights, organizational support, and evidence-based analysis to influence decision-making in alignment with CINQCARE's objectives. The role encompasses the following responsibilities:

  • Collaborate with a team of Practice Transformation Facilitators across various healthcare settings.
  • Assist practices in leveraging information technology to improve patient outcomes and operational efficiencies, focusing on:
    • Strategies for population health management
    • Registry and enrollment processes
    • Care coordination workflows
    • Implementation of care management strategies
    • Development of patient action plans
    • Data collection and reporting for performance monitoring
    • Patient safety protocol implementation
    • Facilitation of community learning forums
  • Act as the primary liaison between healthcare organizations and external stakeholders, facilitating the integration of clinical processes.
  • Guide care teams in adopting best practices and identifying performance gaps through data analysis.
  • Establish and nurture trust-based relationships with healthcare practices to support change management and sustainability of initiatives.
  • Deliver progress reports to practices, highlighting performance gaps and proposing solutions.
  • Prioritize key quality and utilization initiatives essential for success in value-based care programs.
  • Lead practice transformation forums to promote knowledge sharing among stakeholders.
  • Provide training on technology tools to healthcare teams, ensuring integration of new functionalities into workflows.
  • Ensure adherence to regulatory standards, including those set by NCQA.
  • Document and evaluate practice progression throughout the transformation journey.
  • Facilitate effective improvement team meetings to drive continuous enhancement efforts.
  • Connect practices with external resources to support transformation initiatives.
  • Coordinate interactions with other functional areas within the healthcare organization.

Work Environment

This position requires on-site engagement in a traditional office environment, equipped with conditioned air and artificial lighting. Effective communication with various stakeholders, including customers and colleagues, is essential. The role involves regular use of telecommunication and email for correspondence. The ability to sit for extended periods and manage basic office equipment is necessary.
