Key Responsibilities:

  • Help clients understand their health condition(s) and develop strategies to improve their health and wellbeing.
  • Facilitate access to quality services and health resources by serving as a liaison between health/social services and the community.
  • Assist clients with insurance applications and provide informal counselling, support, and follow up as needed.
  • Collect data and report findings to healthcare providers.

Requirements:

  • High School Diploma/GED - CHW Certification preferred with one or more years of experience.
  • Must agree with and sign LifePath's Statement of Christian faith.
  • Proficiency with MS Office Suite, computer input, and database management.

Working Conditions:

  • Inside and outside environment.

Salary: $18/hr
