Key Responsibilities:
- Provide direct patient care, including administering medications, wound care, and vital sign monitoring
- Develop and implement individualized care plans that cater to each patient's unique needs and preferences
- Collaborate with interdisciplinary teams to ensure seamless coordination of care and excellent outcomes

Requirements:
- Current licensure as a Licensed Practical Nurse (LPN) in the state of New York
- 2+ years of experience in long term care or a related field
- Strong communication and interpersonal skills, with the ability to work effectively in a fast-paced environment
- Ability to lift up to 50 pounds and stand for long periods of time
