Job Summary

We are seeking a highly motivated and compassionate individual to join our team as a Family Service Worker. In this role, you will work closely with families to provide support and guidance, helping them to overcome challenges and achieve their goals.

Key Responsibilities
  • Conduct intake and initiate necessary forms to open a case
  • Establish participant rapport and trust, providing empathetic encouragement and support
  • Conduct initial assessments in collaboration with Clinical Specialists
  • Develop family service plans focusing on child development goals and addressing reduction of stress
  • Administer child development screens and other assessments as identified
  • Maintain a list of collaborative contacts and community services pertinent to participant needs
  • Provide information and referral services to community resources and agencies
  • Transport participants as needed, using agency vehicles or personal vehicles
Requirements
  • Up to and including high school education
  • Basic education to understand and follow standard practice or oral and written instructions
  • Valid Hawaii driver's license and clear driver's abstract
