Key Responsibilities:

  • Coordinate care: Develop and oversee care plans for patients in hospice, ensuring their needs are met comprehensively.
  • Conduct assessments: Regularly assess patients' needs, symptoms, and overall condition to develop and adjust care plans accordingly.
  • Provide support: Offer support and guidance to patients and their families, addressing concerns and providing education about the hospice process.
  • Collaborate with the team: Work closely with physicians, other healthcare professionals, social workers, and counselors to ensure a holistic approach to patient care.

Requirements:

  • RN license in state of practice
  • Hospice experience highly preferred
