Job Title: Mental Health Case Manager

About the Role:

We are seeking a dedicated and compassionate Mental Health Case Manager to join our team at Bright Harbor Healthcare. As a key member of our crisis diversion program, you will play a vital role in supporting individuals experiencing mental health crises and connecting them with essential resources and services.

About You:

To succeed in this role, you should have a strong background in human services, excellent communication skills, and the ability to work effectively with diverse populations. A Bachelor's Degree in a related field is required.

Responsibilities:

  • Provide supportive counseling and connect individuals with ongoing behavioral health services and community supports as needed.
  • Conduct regular assessments of clients' well-being, clinical symptomology, and medication compliance.
  • Assist clients with daily living tasks through modeling and direct support.
  • Offer crisis intervention services and respond to emergencies as needed.

What We Offer:

  • A competitive salary range of $60,000 - $80,000 per year, depending on experience.
