Job Summary

The Registered Nurse (RN) will provide care and services to a group of patients, visiting them in their homes, skilled nursing facilities, and assisted living facilities. As a contributing member of the interdisciplinary team, the RN will provide expertise on psych-social assessments, interventions, and community resources.

Responsibilities
  • Provide care and services to patients in their homes, skilled nursing facilities, and assisted living facilities.
  • Conduct psych-social assessments and develop interventions to meet patient needs.
  • Collaborate with the interdisciplinary team to develop and implement patient care plans.
  • Work closely with patients and their families to ensure their needs are met.
  • Manage multiple priorities and tasks, including documentation, meetings, and communication with healthcare professionals.
Requirements
  • Current Registered Nurse (RN) Licensure: Hold a current RN license in good standing in the state(s) of practice.
  • Teamwork and Communication Skills: Work effectively within a team environment to collaborate with nurses, social workers, chaplains, volunteers, and CNA leadership.
  • Time Management and Organization: Manage multiple priorities and tasks, including documentation, meetings, and communication with healthcare professionals.
  • Electronic Medical Records System: Ability to learn and utilize an electronic medical records system.
  • Reliable Transportation: Possess a valid driver's license, auto insurance, and reliable transportation to travel across the service area.
