Tremont Health & Rehabilitation Center is seeking a skilled and compassionate Certified Occupational Therapy Assistant to join our team in the Memory Care Unit. As a COTA, you will play a vital role in helping patients regain their physical function and achieve their goals.
Key Responsibilities:
- Work under the supervision of a licensed Occupational Therapist to develop and implement treatment plans for patients in the Memory Care Unit.
- Conduct assessments and evaluations to identify patients' needs and develop individualized treatment plans.
- Provide occupational therapy services to patients, including exercises, activities, and education to promote independence and functional ability.
- Collaborate with the healthcare team to ensure comprehensive care and coordination of services.
- Maintain accurate and detailed records of patient progress and treatment plans.
- Communicate effectively with patients, families, and healthcare team members to ensure seamless care and coordination.
Requirements:
- Associate's degree from an accredited college in Occupational Therapy Assistant Studies.
- Current and valid license as a Certified Occupational Therapy Assistant (COTA) in the state of employment.
- Previous experience working in a long-term care setting, preferably in a Memory Care Unit.
- Strong communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare team members.
- Ability to maintain confidentiality and handle sensitive information with discretion.
