Job Description

We are seeking an experienced Case Manager to join our team in Missouri City, TX. As a key member of our interdisciplinary team, you will play a vital role in coordinating patient care and ensuring seamless transitions between healthcare settings.

Key Responsibilities
  • Supervise and manage all aspects of patient care, including LVN and HHA supervision.
  • Coordinate the distribution of visits among all disciplines (SN, PT, OT, ST, MSW, HHA) after the Start of Care, Recertification or Resumption of Care is completed.
  • Facilitate interdisciplinary communication to ensure that patients' needs and expectation for services provided are understood and to ensure optimization of service delivery and patient progress.
  • Lead weekly interdisciplinary case conferences for patient caseload, and contribute to the development of care plans for patients.
Requirements
  • Education: Registered Nurse, licensed per state requirements.
  • Experience: 2 years of experience in Home Health preferred.
  • Skills: Good communication skills, assessment skills, and strong clinical documentation skills.
