Job Title: Licensed Practical Nurse

Community Interactions, Inc. is a non-profit organization dedicated to providing services for individuals with intellectual, physical, and behavioral health disabilities.

Job Summary:

The Licensed Practical Nurse (LPN) is a professional, medical position supporting individuals with disabilities receiving services through Community Interactions, Inc. The responsibilities of this position include assisting individuals in achieving their desired outcomes, performing general office duties, and working outside of the office setting.

Key Responsibilities:
  • Assist individuals in achieving their desired outcomes as identified in their Annual Plan
  • Perform general office duties, including writing, typing, filing, and communicating with others
  • Work outside of the office setting, visiting homes and other work sites
  • Travel to program sites and meetings
  • Coordinate discharge plans with hospital staff
  • Assure that adequate supports are in place for individuals once hospital discharge has occurred
  • Attend planning meetings to provide medical information and advocate for individuals' healthcare needs
Qualifications:
  • Must hold a Practical nurse licensed in the state of Pennsylvania
  • Three years minimum experience in the nursing field
  • Experience working with individuals with disabilities is preferred
