Job Summary

Potomac Healthcare Solutions is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will play a critical role in providing pharmaceutical care to patients and supporting the healthcare team in delivering high-quality patient care.

Key Responsibilities
  • Pharmaceutical Care: Provide comprehensive pharmaceutical care to patients, including assessing medication therapy, monitoring patient responses, and making recommendations for medication adjustments.
  • Collaboration with Healthcare Team: Work closely with physicians, nurses, and other healthcare professionals to ensure seamless patient care and optimize medication therapy.
  • Medication Therapy Management: Develop and implement medication therapy plans, including ordering and evaluating laboratory tests, and monitoring patient outcomes.
  • Education and Training: Provide education and training to patients, healthcare professionals, and other stakeholders on medication use, safety, and best practices.
  • Quality Improvement: Participate in quality improvement initiatives to enhance patient care, medication safety, and overall healthcare outcomes.
Requirements
  • Education: Doctorate degree in Pharmacy from an accredited institution.
  • Experience: Minimum of one year of professional pharmacy experience within the last two years.
  • Licensure: Current, full, active, and unrestricted license to practice as a pharmacist.
  • Certifications: BLS CPR certification.
