Job Summary

Potomac Healthcare Solutions is seeking a highly skilled Clinical Pharmacist to join our team. As a Clinical Pharmacist, you will be responsible for providing pharmaceutical care to patients in a dynamic and rapidly growing healthcare environment.

Key Responsibilities
  • Pharmaceutical Care: Assess patient's response to drug therapy and plan drug therapy based on physician-established diagnoses.
  • Medication Management: Order and evaluate laboratory tests necessary to evaluate drug therapy effect and outcomes.
  • Therapeutic Services: Initiate, modify, or discontinue medications for ongoing therapy of chronic disease states.
  • Pharmacotherapy Management: Monitor and manage pharmacotherapy requiring periodic adjustment due to specific or changing pharmacokinetic characteristics.
  • Medication Administration: Administer prescription or non-prescription drugs according to established agreements or protocols.
  • Medical History Review: Evaluate medical and medication histories for drug-related problems and adjust drug therapy accordingly.
  • Consultation Services: Consult with other healthcare providers about patient treatment needs or options.
  • Medication Review: Review patient profiles at the time of dispensing to closely monitor medication therapy.
  • Quality Assurance: Ensure prescription directions for use, dose, medication interactions, therapeutic duplications or overlaps, allergy information, age-specific dosage, and other pertinent data are appropriate for individual patients.
  • Training and Education: Provide medication and healthcare-related in-service training to clinical staff on new medications, appropriate use of medications, comparison of current medication therapies, and other topics as deemed appropriate by the department head.
  • Collaboration and Communication: Consult with other specialty practitioners to provide optimal patient pharmaceutical care.
  • Documentation and Reporting: Document significant medication interactions, and pharmacy interactions with prescribers. Conduct Adverse Drug Reaction (ADR) reporting in accordance with The Joint Commission (TJC) or the Accreditation Association for Ambulatory Health Care, Inc. (AAAHC) standards as appropriate.
  • Administrative Tasks: Perform administrative tasks, including answering phones and providing professional customer service to patients and hospital staff.
Requirements
  • Education: Doctorate Degree in Pharmacy from a college or university accredited by the Accreditation Council on Pharmaceutical Education (ACPE).
  • Experience: Minimum of one year of professional pharmacy experience within the last two years.
  • Licensure: Current, full, active, and unrestricted license to practice as a pharmacist.
  • Certification: BLS CPR certification.
  • Citizenship: Must be a US Citizen.
