About the Role

The Social Work Care Manager is a critical member of our care team, responsible for coordinating patient care and case management services for individuals with complex psychosocial needs. This role requires a strong understanding of social determinants of health and the ability to partner with patients, families, and healthcare providers to develop and implement effective care plans.

Key Responsibilities
  • Conduct comprehensive social assessments to identify patient needs and develop personalized care plans
  • Collaborate with patients, families, and healthcare providers to implement care plans and address social determinants of health
  • Provide education and support to patients and families on healthcare navigation, resource utilization, and health literacy
  • Develop and maintain relationships with community resources and organizations to ensure seamless care coordination

Requirements
  • Bachelor's degree in social work, public health, or a related field
  • 3+ years of experience in healthcare or social services, preferably in a care coordination or case management role
  • Clinically licensed as an LMSW in the state of Georgia
  • Knowledge of local community resources and entitlement processes
  • Strong written and verbal communication skills, with ability to work effectively in a team environment
