Atrium Health Navicent is seeking a highly skilled and compassionate Social Worker to join our team as an Acute Care Navigator. In this role, you will play a critical part in facilitating patient care, providing ongoing care management services, and promoting efficient utilization of resources.
Job Summary:
As an Acute Care Navigator, you will be responsible for coordinating and supporting the providers and multidisciplinary team in facilitating patient care. Your primary objective will be to enhance the quality of clinical outcomes and patient satisfaction while managing the cost of care.
Key Responsibilities:
- Participate in rounds on the patient care unit with the attending physician and other members of the healthcare team to ensure collaboration and consistency in moving the patient's care to estimated date of discharge.
- Assess patients to determine their discharge planning and/or post-acute transition needs, and develop a discharge plan that addresses their physical, functional, social, and psychological status, as well as their cultural and language needs.
- Assign the appropriate care pathway based on clinical feedback from the physician and diagnosis-DRG, and ensure coordination of services among the patient's physicians, specialists, community agencies, and vendors.
- Assess progress toward goals and identify barriers to meeting goals, and prepare and maintain appropriate documentation of patient care and progress within designated systems.
Requirements:
- Bachelor's degree in Social Work, Master's preferred.
- Three years of recent experience in acute care, home health, case management, discharge planning, or care management.
- Discharge Planning in a healthcare setting preferred.
- Experience with IT solutions such as electronic health records, learning management, or disease/care management systems a plus.
