Job Summary

We are seeking a highly skilled Clinical Linkage Coordinator to join our team at Family Residences and Essential Enterprises, Inc. The successful candidate will be responsible for providing crisis support, linkages, outreach, and consultation services for individuals with developmental disabilities and behavioral health needs.

Key Responsibilities

  • Collaborate with individuals' systems of support to provide systemic consultation and develop person-centered services.
  • Work with external partners and internal resources to create comprehensive service evaluations and cross-systems crisis prevention and intervention plans.
  • Prepare agendas and document outcomes for individual mentoring sessions with the Center for START Services.
  • Maintain linkages and relationships with community partners to ensure seamless service delivery.
  • Assist with referrals for consultation and treatment as needed, and provide outreach visits to individuals' homes, day, and vocational settings.
  • Develop and implement Comprehensive Service Evaluations, Cross-Systems Crisis Prevention and Intervention Plans, intake/assessments, intervention and outcome plans for Resource Center admissions or inpatient hospital admissions.
  • Manage an active caseload of individuals referred for START Services, ensuring the coordination of support meetings and crisis plan development.
  • Participate in recurring meetings with START leadership, clinical team, and Resource Center program, and engage in National Center for START Services trainings and events.
  • Address emergency situations according to established policies and procedures, and maintain on-call responsibilities as required.

Requirements

  • Master's Degree in Social Work, Psychology, Counseling, or other human service field.
  • 1 year's experience working with individuals with IDD and mental health and/or behavioral health needs.
  • START Clinical Linkage Coordinator certification must be completed within 12 months.

Work Environment

This role is performed in-person, with excessive travel from one local site to another within the FREE Network possible. The employee is required to sit, walk, and stand for prolonged periods, effectively communicate verbally and in writing, and lift 50 pounds at a time. Reasonable accommodations will be provided to allow employees to perform the essential functions of the position.
