Key Responsibilities- Provide daily support and assistance to individuals with skills training in activities of daily living, including personal care, meal preparation, and household management.
- Create opportunities for individuals to make decisions and develop their skills, fostering independence and self-advocacy.
- Ensure compliance with regulatory standards, including DDS, DMH, MassHealth, HUD, DPH, and CARF, as well as Bridgewell policies and procedures.
- Provide medical, clinical, and financial support to individuals, including transportation to medical appointments and activities.
- Maintain accurate and detailed documentation, adhering to regulatory requirements and Bridgewell policies.
- Develop positive relationships with guardians, family members, and other stakeholders, ensuring effective communication and collaboration.
- High school diploma or equivalent required; Associate's degree or DSP certification preferred.
- Minimum 1 year of experience working with individuals with developmental or psychiatric disabilities.
- Excellent written and oral communication skills, with ability to maintain accurate documentation.
- Valid driver's license and minimum 1 year of driving experience.
- Computer literacy, including proficiency in Microsoft products and web-based applications.
- Frequent climbing of stairs.
- Lifting a minimum of 50 pounds.
- Regular driving and meeting driving record requirements.
- Normal range of eyesight and hearing.
