About the Role:The Licensed Vocational Nurse will be responsible for providing high-quality patient care and performing technical skilled care in the patient's home. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.
Responsibilities:- Provide quality patient care and perform technical skilled care in the patient's home.
- Develop and implement individualized care plans in collaboration with the healthcare team.
- Conduct thorough assessments of patients' physical and emotional needs.
- Administer medications and treatments as prescribed by physicians.
- Monitor patients' progress and report any changes or concerns to the supervising nurse and/or physician.
- Instruct patients and their families on proper care and maintenance of a healthy environment.
- Current LPN licensure in the state of practice.
- Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
- CPR Certification required.
