Care Partner

At Sonnet Hill Senior Living, we are seeking a compassionate and dedicated Care Partner to join our team. As a Care Partner, you will play a vital role in providing exceptional care and support to our residents, ensuring their physical, emotional, and social needs are met.
Responsibilities:- Assist residents with activities of daily living, including bathing, dressing, and grooming.
- Provide emotional support and companionship to residents, promoting a sense of belonging and well-being.
- Assist with medication management, including administering medications and monitoring side effects.
- Participate in resident care planning, working collaboratively with the care team to develop and implement individualized care plans.
- Maintain accurate and detailed records of resident care, including progress notes and medication administration records.
- Assist with housekeeping and laundry duties, ensuring a clean and comfortable living environment for residents.
- Respond to resident emergencies, including falls, medical emergencies, and behavioral crises.
- Participate in staff meetings and training sessions, staying up-to-date on best practices and industry developments.
- High school diploma or equivalent required; degree in a related field (e.g., nursing, social work) preferred.
- Previous experience in a care-giving role, preferably in a senior living setting.
- Ability to work independently and as part of a team, prioritizing resident needs and collaborating with colleagues to achieve shared goals.
- Excellent communication and interpersonal skills, with the ability to build strong relationships with residents, families, and staff.
- Ability to maintain confidentiality and handle sensitive information with discretion.
- Physical ability to perform tasks that require standing, walking, and lifting (up to 80 lbs.).
