Job Summary:The LPN will be responsible for delivering skilled nursing care to patients in accordance with their individualized care plans and physician orders. This includes assessing patient needs, developing and implementing care plans, and providing education to patients and their families.
Key Responsibilities:- Deliver high-quality patient care in compliance with physician orders and care plans.
- Assess patient needs and develop individualized care plans.
- Provide education to patients and their families on disease management, medication, and other health-related topics.
- Collaborate with the healthcare team to ensure seamless care coordination.
- Document patient progress and communicate with the healthcare team as needed.
- Active LPN license in the state of application.
- Valid CPR certification.
- Valid driver's license.
- Medicare skilled nursing experience.
- Basic understanding of Oasis.
- 1-year LPN experience in a home health or healthcare setting.
