Position Overview

The Hawai'i Health & Harm Reduction Center (HHHRC) is dedicated to providing essential harm reduction services to underserved communities in Hawai'i, including individuals impacted by HIV, hepatitis, substance use, homelessness, and those from the transgender, LGBQ, and Native Hawaiian populations. Our mission is to facilitate access to vital resources such as housing, healthcare, entitlements, and treatment while promoting health, wellness, and systemic change through outreach, advocacy, education, and capacity building.

ROLE SUMMARY

The Sexual Health Navigator plays a crucial role in delivering outreach, risk-reduction counseling, and screening services for HIV, Hepatitis C, and STIs, with a focus on individuals at heightened risk. This position is also responsible for providing follow-up counseling and referrals for clients who are HIV positive or at high risk of infection, guiding them towards Pre-exposure Prophylaxis (PrEP) and other preventive services.

REPORTING STRUCTURE:

Reports to: HIV Program Director

Supervises: N/A

KEY RESPONSIBILITIES:

Testing and Linkage Services:

  • Conduct HIV/Hepatitis C/STI testing and linkage services in both office and community settings.
  • Provide harm reduction counseling to individuals utilizing testing and linkage services.
  • Assist clients in navigating PrEP and health insurance options as necessary.
  • Facilitate appropriate referrals for additional services.
  • Administer vaccinations and perform blood draws.

Community Outreach:

  • Engage in community outreach with at-risk populations, including Native Hawaiian, Asian, Pacific Islander communities, men who have sex with men (MSM), and transgender individuals, in various venues such as:
  1. Social gathering spots (e.g., bars, nightclubs).
  2. Online platforms and mobile applications popular among target populations.
  3. Social media channels.
  4. Locations where HHHRC's Medical Mobile Unit operates.
Implement existing HIV risk-reduction programs to enhance awareness and minimize transmission risks within these communities:
  1. Utilize culturally sensitive harm reduction techniques in diverse settings.
  2. Settings may include parks, beaches, and private residences.

Additional Duties:

  • Support the design, implementation, and evaluation of HIV/HCV/STI interventions:
  1. Contribute to the creative process for developing engaging educational materials.
  2. Stay informed on the latest HIV/STI/HCV information from credible sources.
  3. Keep up-to-date with effective HIV prevention strategies relevant to the target population.
Collaborate with local agencies and organizations to enhance access to testing, PrEP, and other preventive services:
  1. Lead meetings with external partners.
  2. Assist in project implementation and delegate tasks to community members.
  3. Participate in relevant planning groups.
Ensure compliance with all organizational record-keeping and reporting requirements.Perform other duties as assigned, which may evolve over time.

QUALIFICATIONS:

Knowledge, Skills, and Abilities:

  • Proficient in Microsoft Office Suite.
  • Strong verbal and written communication skills.
  • Ability to engage effectively with diverse populations.
  • Experience with mobile applications and social media platforms.
  • Commitment to maintaining client confidentiality.
  • Flexibility to work non-traditional hours as needed.

Education and Experience:

  • High School diploma or equivalent experience.
  • One or more years of experience working with target populations.

PREFERRED QUALIFICATIONS:

  • Associate's or Bachelor's degree in Social Work or a related field.
  • In-depth knowledge of HIV/AIDS and harm reduction strategies.

CERTIFICATIONS:

  • Valid Hawaii Driver's License.
  • COVID-19 Vaccination.
  • TB Clearance.
  • Hepatitis B Clearance and/or Vaccination.
