Community Health Worker of the HIV/HEIS Program Specialist

Waikiki Health Center is seeking a highly motivated and compassionate Community Health Worker of the HIV/HEIS Program to join our team. As a key member of our interdisciplinary team, you will provide quality care to patients living with HIV/AIDS and affected others to achieve optimal clinical outcomes.
Responsibilities- Assist in providing services during clinic hours, participating in huddles, population-based management, patient intakes, education, HIV rapid testing and counseling, within clinic protocols.
- Collaborate with multiple WH providers, other support staff, healthcare data analyst, insurance agencies, partner agencies, and diverse patient populations.
- Empower patients with education on basic health information, health care coverage options, and access to other health and community resources.
- Assess financial eligibility for Ryan White services, assist with enrollment in WH's Ryan White program, and provide education on participant requirements and eligible services.
- Provide HIV, Hepatitis C, Gonorrhea, and Chlamydia counseling and rapid testing in clinical settings in compliance with strict standards of Waikiki Health, the State of Hawaii Department of Health, and the Centers for Disease Control.
- Transportation and valid driver's license.
- Familiarity with common software programs, (i.e. Windows-based programs) an ability to quickly learn new systems.
- Ability to communicate effectively with patients, community members, and staff members.
- Proficient in computer skills: typing, navigating through the internet and emails.
- OSHA training.
- BCLS biannually.
