Amedisys is seeking a skilled and compassionate Licensed Practical Nurse (LPN) to join our team as a Home Health Care Specialist. As a key member of our care team, you will provide high-quality patient care in the comfort of their own homes.
Responsibilities- Assess and Plan Care: Verify patient plans of care with the RN prior to each visit and collaborate with the physician and RN to ensure patient needs are being met.
- Deliver Patient Care: Teach patients and caregivers self-care techniques, check vital signs, review medications, and perform specialized procedures as ordered by the physician.
- Document Patient Information: Complete documentation timely and accurately, including patient progress and any changes in their condition.
- Experience: One year of clinical LPN experience.
- Licensure: Current LPN license, specific to the state(s) you are assigned to work.
- Certifications: Current CPR certification and valid driver's license.
