RN Clinical Manager, Home Health

About the Role:
As a RN Clinical Manager, Home Health, you will play a vital role in leading our team of healthcare professionals in providing high-quality patient care in the comfort of their own homes. You will be responsible for coordinating and overseeing all direct care patient services, ensuring that our patients receive the best possible care and support.
Key Responsibilities:
- Develop and implement clinical operations for a specific location, ensuring compliance with regulatory requirements and company policies.
- Conduct assessments and reassessments of patients, updating care plans and interpreting patient needs to ensure the best possible outcomes.
- Manage the assignment of caregivers, ensuring that patients receive the right care at the right time.
- Oversee the delivery of care to all patients served by the location, ensuring that care is delivered in a timely and effective manner.
- Collaborate with clinicians to promote effective performance and delivery of quality home care services.
- Participate in performance improvement activities, maintaining ongoing clinical knowledge through internal and external training programs.
- Develop and maintain relationships with referral sources and community partners to promote our services and support our patients.
Requirements:
- Graduate of an accredited School of Nursing.
- Active/unrestricted state license as a Registered Nurse.
- Proof of current CPR.
- Valid driver's license, auto insurance, and reliable transportation.
- Two years as a Registered Nurse with at least one year of management experience in a home care, hospice, or equivalent environment.
Preferred Qualifications:
- Home Health working experience.
- Direct management experience leading a multidisciplinary team with a minimum of 7 employees.
- Experience with Homecare Homebase (HCHB) software and OASIS.
- Strong clinical background in home care operations and quality assurance.
