Clinical Care Coordinator

The Clinical Care Coordinator will work closely with physicians and other healthcare professionals to develop and implement individualized care plans for patients. This role requires strong communication and interpersonal skills, as well as the ability to work effectively in a fast-paced environment.
Key Responsibilities:
• Perform continuous assessments and evaluations to ensure patients are progressing towards desired outcomes.
• Assess and respond to patient/family needs by coordinating efforts of other team members.
• Identify and resolve barriers that hinder effective patient care.
• Collaborate and consult with physicians on patient's progress and discharge planning needs.
• Perform utilization review ensuring admissions meet criteria for appropriateness of care and medical necessity.
• Manage the process to review and, as appropriate, appeal denials received from payors.
• Coordinate the discharge planning process ensuring involvement of all members of the healthcare team.
• Counsel with patients and family members in decision making and in meeting psychosocial needs of the patient.
• Collaborate with personnel at other facilities to coordinate smooth and effective patient transfers and transitions.
• Compile, evaluate, and report statistics to members of the team and utilize the information to facilitate process improvement activities.
• Support and participate in hospital safety programs, infection control and other processes to ensure a safe environment.
Requirements:
This role requires a Bachelor's degree and licensure as a vocational or registered RN. A minimum of 5-10 years previous case management experience is preferred, as well as basic knowledge of CMS guidelines and experience working with payor specific guidelines and contractual rules. The ideal candidate will possess excellent customer service skills, critical thinking skills, and the ability to work with minimal supervision meeting firm deadlines.
Work Environment:
The Clinical Care Coordinator will work in a fast-paced healthcare environment, frequently required to sit, converse, and listen; use hands to touch, handle, or feel objects, tools or controls; and to reach with hands and arms. Specific vision abilities required include close vision and the ability to adjust focus. The employee must be able to lift and/or carry over 15 pounds on a regular basis and be able to push/pull over 15 pounds on a regular basis. The employee must be able to stand and/or walk at least five hours per day.
