Job Summary:

Cantex Continuing Care Network seeks a skilled Licensed Vocational Nurse to provide high-quality patient care in the comfort of their own homes. As a key member of our interdisciplinary team, you will work closely with patients, families, and healthcare professionals to develop and implement personalized care plans that promote optimal health and well-being.

Responsibilities include administering skilled nursing care, coordinating care with the interdisciplinary team, and collaborating with patients and caregivers to achieve their goals. Key Responsibilities:

  • Administer skilled nursing care to patients in their place of residence
  • Coordinate care with the interdisciplinary team and patient/caregivers
  • Collaborate with physician, clinical team, and patient in preparation for discharge
  • Participate in weekly case conferences and interdisciplinary communication
  • Provide verbal or written patient status updates to supervising RN

Requirements:

  • Licensed Vocational Nurse, per state requirements
  • Good communication skills, assessment skills, and strong clinical documentation skills
  • Ability to read, write, and speak English
  • Reliable transportation and valid auto liability insurance
