We are seeking a dedicated Member Care Coordinator to enhance our member engagement efforts.
Key Responsibilities:
- Manage assessments and updates within the home health system.
- Conduct outreach and follow-up with members regarding their healthcare plans.
- Foster relationships with members to promote adherence to care plans and promptly notify Case Managers of any arising issues.
- Educate members about their programs, utilizing provided scripts as necessary.
- Perform data entry to maintain accurate records in the system.
- Encourage members to utilize our services, including scheduling appointments and additional support services.
- Generate and send correspondence to members through various communication methods.
- Conduct regular check-ins with members to assess progress towards individual care plan goals.
- Meet production targets aligned with departmental needs.
- Support the clinical team by executing non-clinical tasks essential for case management.
- Respond to inquiries via phone or written communication, ensuring member needs are addressed.
- Update member or provider information through data entry.
- Collect necessary information through correspondence or phone calls.
- Act as a liaison for various groups regarding claims, conducting research and obtaining required documentation.
- Maintain effective communication with internal departments regarding group concerns.
Qualifications:
- Bachelor's degree in Social Work or Psychology, or an LVN/LPN with one year of experience in managed care systems, or an RN, or three years of care coordination experience in a state-managed or waiver program, or three years of experience in managed care systems.
- Familiarity with medical terminology.
- Experience in coordinating medical needs and providing member assistance.
- Proficiency in Microsoft Office applications.
- Strong customer service skills.
- Excellent verbal and written communication skills, including the ability to develop written correspondence.
- Coaching skills, including motivational interviewing techniques.
- Valid state driver's license, transportation, and applicable insurance.
- Willingness to travel as needed.
