Position Overview

The Patient Care Management Nurse plays a crucial role in overseeing the comprehensive care and services provided to patients. This position emphasizes quality care through a collaborative approach that effectively coordinates, monitors, and assesses services tailored to the individual needs of patients.

Key Responsibilities
  • Collaborate with patients, their families, and the healthcare team to ensure a seamless transition from one care setting to another.
  • Adhere to all relevant laws, regulations, and organizational standards while delivering care.
  • Utilize a nursing diploma (either an associate's or bachelor's degree) as a foundation for clinical practice.
  • Bring at least one year of clinical experience in a post-acute care environment.
  • Preferably possess experience in case management, utilization review, and discharge planning.
  • Hold certifications such as Certified Case Manager (CCM) or Board Certification in Nursing Case Management (RN BC) as an advantage.
  • Exhibit expert knowledge in nursing practices and procedures.
  • Stay informed about the regulations and guidelines governing operations within post-acute care facilities.
  • Implement and interpret departmental programs, goals, objectives, policies, and procedures.
  • Demonstrate proficiency in all competency areas, including patient rights, safety, and sanitation.
  • Maintain confidentiality regarding all proprietary and sensitive information.
  • Comply with company policies, including those related to harassment and compliance.
  • Uphold integrity and professionalism by adhering to the organization's Code of Conduct and completing necessary compliance training.
  • Efficiently utilize relevant computer systems for data entry and retrieval.
  • Act as a liaison between external case managers, families, physicians, and community resources.
  • Provide training and education to patients, families, associates, and other care providers.
  • Implement standards of practice for care management, ethical performance, and coordination of care.
  • Demonstrate proficiency in reading, writing, speaking, and understanding the English language.
