Patient Care Coordinator

- Room patients, obtain vital signs, and collect patient data in the clinic or over the phone.
- Follow up on patient issues, assist with appointments, and communicate care instructions.
- Provide interpreting and translating services as needed.
- Maintain documentation of patient contacts and enter data into the electronic medical record.
- High School diploma or equivalent required; 1 year experience in human services or healthcare preferred.
- Effective verbal and written communication skills, with ability to speak a second language (Spanish).
- Basic computer skills and proficiency using Microsoft Office applications.
- Data entry skills and customer service experience.
