Job Overview

Duties and Responsibilities
  • Evaluate member needs and recommend optimal plans for the best outcome.
  • Develop ongoing long-term care plans and identify necessary providers, specialists, and community resources.
  • Coordinate between members/families/caregivers and the care provider team to ensure accessible services.
  • Provide resource support for various needs based on service assessment and plans.

The estimated salary range for this position is $25.15 - $41.00 per hour, based on individual skills, experience, education, and other job-related factors permitted by law.
