Clinical Care Coordinator

**Job Overview:**
We are seeking a skilled Licensed Practical Nurse (LPN) to join our team at Primary Health Care Inc. as a Clinical Care Coordinator.
This is a fantastic opportunity for an LPN to take on a challenging role in a dynamic healthcare environment.
Responsibilities:
- Provide high-quality patient care and support services, including clinical documentation, patient rooming, and vital signs collection.
- Administer medications, immunizations, and treatments according to written orders.
- Perform lab and diagnostic testing, explain procedures to patients, and report results accurately.
- Facilitate prior authorizations, referrals, and other administrative tasks.
Requirements:
- Current LPN license accepted by the Iowa Board of Nursing.
- Basic Life Support (BLS) certification for Health Care Providers.
- Mandatory Reporter Certification.
Compensation:
The estimated salary for this position is $22.30 - $27.33 per hour, with opportunities for additional compensation based on experience.
