Job Title: Nurse Care Manager
">Estimated Salary Range: $85,000 - $110,000 per year, depending on experience.
">About the Role:
">We are seeking a skilled and compassionate Nurse Care Manager to join our team at Santa Rosa Medical Center. As a key member of our healthcare team, you will play a critical role in ensuring that our patients receive high-quality, patient-centered care throughout their hospital stay and beyond.
">Key Responsibilities:
">- Act as a liaison between patients/families and healthcare personnel to ensure necessary care is provided promptly and effectively.
">- Rely on experience and judgment to plan and accomplish goals, promoting a smooth transition from hospital to home or other post-acute settings.
">- Collaborate with interdisciplinary teams to develop and implement effective discharge plans, taking into account each patient's unique needs and circumstances.
">- Communicate daily with admissions personnel regarding admissions and discharges to various units, ensuring seamless transitions and minimizing delays.
">- Evaluate and screen potential admissions to the facility, making recommendations for admission based on clinical criteria.
">- Initiate ongoing communication with residents and their families to assess discharge needs, providing education and support throughout the process.
">- Document discharge planning as an ongoing review, tracking progress and making adjustments as needed.
">- Work closely with physicians to ascertain their plans for timely discharge, ensuring that all necessary arrangements are made.
">- Cooperate with insurance companies, based on information received, to facilitate smooth billing and reimbursement processes.
">- Manage and collaborate with the healthcare team on Swing Bed placement, completing MDS documentation as necessary to ensure accurate assessment and care planning.
">- Ensure that a quality of care is maintained or surpassed by collecting quality indicators and variance data, reporting findings to the appropriate department and identifying areas for improvement.
">- Assist as needed with obtaining referrals, prior authorization for Home Health Care, DME, SNF, acute rehab, and appointments, streamlining the referral process for patients and families.
">- Educate physicians and staff regarding appropriate level of care/utilization issues, promoting informed decision-making and best practices within the organization.
">Requirements and Qualifications:
">- Active Florida RN License
">- 2+ years of Case Management experience
">- 2+ years of Utilization review experience
">- Knowledgeable of resident's financial status, diagnosis, and discharge needs
">- Familiar with standard concepts, practices, and procedures within a particular field
">- Strong communication and interpersonal skills, with ability to work effectively with diverse stakeholders
">- Ability to analyze complex data and make informed decisions, promoting quality care and efficient resource allocation
