About the Role

We are seeking a compassionate and skilled Hospice Registered Nurse to join our team at Emerald Coast Hospice. As a RN Case Manager, you will play a vital role in providing exceptional patient care and support to individuals and their families during times of need.

The ideal candidate will possess strong communication skills, empathy, and a passion for delivering high-quality care in a home-based setting. You will work closely with our interdisciplinary team to develop and implement individualized plans of care, ensuring our patients receive comprehensive support and comfort throughout their journey.

Responsibilities
  • Provide direct patient care services, including assessing patient needs, developing and implementing plans of care, and coordinating with healthcare teams.
  • Act as a primary liaison between physicians, patients, and their families, ensuring seamless communication and collaboration.
  • Maintain accurate and detailed clinical records, adhering to regulatory requirements and industry standards.
  • Collaborate with the hospice team to provide emotional and spiritual support to patients and their loved ones.
Requirements
  • RN license in the state of employment (or eligibility to obtain).
  • 1+ years of nursing experience in hospice, home health, ICU, geriatric, or oncology strongly preferred.
  • Demonstrate flexibility, composure, and compassion while supporting patients and families in crisis situations.
  • Current driver's license and automobile insurance required.
  • CPR certification is essential.
