Clinical Nurse Team Lead

Barnes-Jewish Hospital is seeking a skilled Clinical Nurse Team Lead to join our Neuro Medicine Unit. As a key member of our team, you will be responsible for planning, directing, and coordinating activities of a unit on a designated shift. Your expertise will be invaluable in assessing, planning, evaluating, and documenting the nursing process of assigned patients.
Key Responsibilities:
- Facilitate identification and resolution of gaps in care from hospitalization to discharge and into the ambulatory care setting.
- Critically assess patient data to proactively mitigate impending changes in condition and establish consults to the interprofessional team as appropriate.
- Lead the development and implementation of complex individual plans of care with defined goals in collaboration with other members of the interprofessional team and the patient, family, or caregiver.
Requirements:
- Nursing Diploma/Associate's degree
- 2-5 years of experience in a clinical setting
- Supervisor experience is not required but preferred
