Key Responsibilities:
- Enhancing and/or sustaining residents' abilities to manage their daily physical and mental health needs.
- Assessing individual social and emotional requirements.
- Creating and upholding personalized care strategies.
- Engaging in the planning process for resident admissions, community reintegration, or transfers to alternate facilities.
- Contributing to interdisciplinary care meetings to address the social and emotional aspects of resident care.
- Collaborating with the finance department to ensure residents are informed and that necessary documentation is prepared for their stay.
- Assisting residents with discharge planning and coordinating essential durable medical equipment or home care services to ensure safety upon returning to the community.
- Licensed Master Social Worker (LMSW).
- A minimum of one year of experience in the field of Social Work is essential.
- Experience in long-term care or sub-acute environments is preferred.
- Flexibility in scheduling is required.
- Experience with Person-Centered Care (PCC) is advantageous.
