Job Summary

AdvantageCare Rehabilitation is seeking a compassionate and skilled Certified Nursing Assistant (CNA) to join our Hospice Care team. As a CNA, you will provide high-quality patient care in the comfort of their own homes, making a meaningful difference in the lives of our patients and their families.

About the Role

  • Provide hands-on personal care, including bathing, dressing, and toileting, to patients in their homes.
  • Assist with ambulation and exercises as outlined in the patient's care plan.
  • Observe and report on patient condition, ensuring timely communication with healthcare team.
  • Perform range of motion and other exercises as stated in the care plan, within the scope of your CNA practice.
  • Adhere to company policies and procedures, ensuring a safe and respectful environment for patients and colleagues.
  • Document patient care and progress accurately and thoroughly.

Requirements

  • Current licensure as a Certified Nursing Assistant (CNA) in the Commonwealth of Pennsylvania.
  • Experience with electronic charting and documentation.
  • Valid Pennsylvania driver's license and state-required auto insurance.
  • CPR certification.
  • Home health experience preferred, but not required.
