Job Description

Job Title: Community Impact Director

Job Summary:

The American Heart Association is seeking a highly motivated and experienced Community Impact Director to drive health equity initiatives in our region. As a key member of our team, you will be responsible for developing and implementing collective impact efforts to address social determinants of health, with a focus on hypertension, women's health, nutrition, and tobacco/e-cigarette use in diverse communities.

Key Responsibilities:

  1. Develop and Implement Collective Impact Efforts: Collaborate with internal stakeholders to develop and implement collective impact efforts aligned with our community assessment outcomes.
  2. Build Relationships with Healthcare Partners: Establish and maintain strong relationships with federally qualified health centers, healthcare systems, and hospitals to implement ambulatory care programs focused on hypertension, cholesterol, and diabetes.
  3. Lead Policy, System, and Environmental Changes: Lead the market in developing and executing policy, system, and environmental changes in collaboration with key volunteers and strategic alliances.
  4. Recruit and Develop Diverse Volunteers: Identify and assist in recruiting diverse volunteers to serve in leadership roles across the market.
  5. Secure Program Funding: Collaborate with development staff partners and volunteers to identify, cultivate, and secure program funding.
  6. Align Local Opportunities with National Agenda: Work collaboratively with internal stakeholders to ensure that local opportunities are aligned with the American Heart Association's national agenda.

Qualifications:

To be successful in this role, you will need:

  1. University/College Degree or Equivalent Experience: A degree in public health or healthcare organization, or equivalent experience.
  2. Relevant Experience: 3 years of relevant experience working with community health issues.
  3. Strong Communication Skills: Outstanding oral and written communication skills.
  4. Project Management: Demonstrated ability to handle multiple, complex projects under time constraints.
  5. Technical Skills: Intermediate knowledge of Microsoft Office; advanced knowledge preferred.
  6. Meeting Facilitation: Validated ability to conduct meetings with internal and external clients.
  7. Team Collaboration: Ability to efficiently participate in a multi-disciplinary team environment.
  8. Travel Requirements: Ability to do daily local travel up to 75%; requires access to reliable transportation.
