As a skilled Home Health Nurse, you will play a vital role in delivering high-quality, patient-centered care to individuals in the comfort of their own homes. Your expertise will enable patients to recover, rehabilitate, and regain their independence, empowering them to live healthier and happier lives.

Key Responsibilities:
  • Provide comprehensive nursing care, including admission, case management, and follow-up visits, to ensure patients receive the best possible outcomes.
  • Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to develop and implement personalized care plans.
  • Conduct thorough assessments, identify patient needs, and develop strategies to address them, promoting optimal health and well-being.
  • Administer medications, treatments, and therapies as prescribed, ensuring patients receive timely and effective care.
  • Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and address any concerns or questions.
  • Document patient progress, outcomes, and any changes to care plans, maintaining accurate and up-to-date records.
  • Participate in ongoing education and professional development to stay current with best practices, industry advancements, and regulatory requirements.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum one year of nursing experience, preferably in home health or a related field
  • Strong med-surg, ICU, ER, or acute care experience
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Valid driver's license, auto insurance, and reliable transportation
