Key Responsibilities:
  • Conduct thorough assessments of patients' physical and emotional needs, developing and implementing individualized care plans to promote optimal health and well-being.
  • Provide hands-on care, including medication management, wound care, and rehabilitation services, under the guidance of our experienced clinical team.
  • Collaborate with patients, families, and healthcare providers to ensure seamless care coordination and effective communication.
  • Monitor patients' progress, making adjustments to care plans as needed to ensure the best possible outcomes.
  • Document patient interactions, care plans, and outcomes in a timely and accurate manner, maintaining confidentiality and adhering to regulatory requirements.
Requirements:
  • Diploma, Associate, or Bachelor's degree in Nursing
  • Minimum of one year of nursing experience, preferably in home health or a related field
  • Current and unrestricted Registered Nurse licensure
  • Current CPR certification
  • Strong organizational and communication skills, with the ability to work effectively in a fast-paced environment
