Key Responsibilities:- Provide comprehensive nursing care, including admission, case management, and follow-up visits, to ensure patients receive the best possible outcomes.
- Collaborate with interdisciplinary teams, including physicians, therapists, and social workers, to develop and implement personalized care plans.
- Conduct thorough assessments, identify patient needs, and develop strategies to address them, promoting optimal health and well-being.
- Administer medications, treatments, and therapies as prescribed, ensuring patients receive timely and effective care.
- Communicate effectively with patients, families, and healthcare teams to ensure seamless care coordination and address any concerns or questions.
- Document patient progress, outcomes, and any changes to care plans, maintaining accurate and up-to-date records.
- Participate in ongoing education and training to stay current with best practices, industry developments, and regulatory requirements.
- Diploma, Associate, or Bachelor's degree in Nursing
- Minimum one year of nursing experience, preferably in home health or a related field
- Strong med-surg, ICU, ER, or acute care experience
- Current and unrestricted Registered Nurse licensure
- Current CPR certification
- Valid driver's license, auto insurance, and reliable transportation
