Key Responsibilities:
  • Conduct regular facility rounds to monitor resident care and ensure a safe and clean environment.
  • Supervise the assignments of aides to ensure proper workflow and efficient care delivery.
  • Intervene in resident conflicts, outbursts, and behavior issues to maintain a peaceful and respectful environment.
  • Supervise medication administration times to ensure accurate documentation and follow-up care.
  • Facilitate communication among healthcare team members to ensure timely medical attention.
  • Process late admissions and re-admissions, including paperwork and medication changes.
  • Report to the Director of Nursing (DON) and collaborate with the healthcare team to achieve our goals.
Requirements:
  • Ability to work effectively as a team member.
  • Minimum 5 years of recent clinical nursing experience, with at least 1 year in Long Term Care or sub-acute settings.
  • Valid state LPN license and good standing with the State Registry.
