Job Summary

Under the supervision of the Transportation Coordinator, this role is responsible for the direct transportation of physically limited passengers to and from their destinations. The position requires constant interaction with caregivers and community agencies.

Essential Duties and Responsibilities
  • Effectively communicates with participants, families, staff, and other agencies and departments.
  • Drives a specially equipped van to deliver physically limited persons to varied destinations, ensuring wheelchair lifts and tie-down equipment are properly operated.
  • Follows established schedules, adjusts schedules to meet unanticipated changes, and determines the safest and most efficient pick-up and delivery routes.
  • Assists passengers on and off the van in a safe and courteous manner, dealing with medical equipment and other emergencies that may arise.
  • Inspects the van and maintains it in a clean, orderly, and safe condition, reporting the need for maintenance.
  • Participates as a member of the Interdisciplinary Team, attending staff meetings and taking part in participant care planning.
  • Observes each participant for any change in physical, mental, emotional, and social functioning and reports such changes to the licensed nurse.
  • Completes documentation as required, coordinating 24-hour care delivery.
Requirements
  • High school diploma or equivalent.
  • Minimum 2 years of supervisory experience.
  • Minimum 1 year of experience transporting individuals with special mobility needs.
  • 1 year of experience working with a frail or elderly population preferred.
  • Must have reliable transportation, a valid driver's license, and the minimum state required liability auto insurance.
  • Be medically cleared for communicable diseases and have all immunizations up-to-date before engaging in direct participant contact.
  • Pass a comprehensive criminal background check.
Skills and Abilities
  • Work independently and as an effective member of a team.
  • Good understanding of psychosocial problems of aging.
  • Basic computer skills and use of electronic mail.
  • Be empathic and patient to attend to the needs of a frail elderly population.
  • Communicate clearly and effectively to participants on a daily basis.
  • Proper body mechanics for assisting seniors and persons with mobility limitations.
  • Managing special needs populations, including individuals with dementia.
