Key Responsibilities:
  • Conduct thorough assessments of patients to identify their care needs and develop personalized care plans.
  • Administer medications, perform wound care, and provide other nursing interventions as needed.
  • Collaborate with patients, families, and healthcare teams to ensure seamless care coordination.
  • Document patient progress and outcomes in a timely and accurate manner.
  • Provide education and support to patients and their families on disease management, self-care, and health promotion.
Requirements:
  • Current licensure as a Registered Nurse in the state of North Carolina.
  • Minimum one year of experience as a professional nurse in an acute care setting.
  • Prefer one year of Home Health experience.
  • Valid North Carolina Driver's License.
