Hand n Hand Home HealthCare is seeking a compassionate and skilled Licensed Practical Nurse (LPN) to join their team. As an LPN, you will play a vital role in providing high-quality patient care in the comfort of their own homes.

The ideal candidate will have a strong foundation in clinical skills, including wound care, medication administration, and vital sign monitoring. You will work closely with patients, families, and healthcare teams to develop and implement personalized care plans.

Responsibilities:

  • Assess and chart patient observations and conditions
  • Complete evaluation tasks, including medication reviews and vital sign monitoring
  • Administer physician-prescribed medication and provide education to patients and families
  • Dress and redress wounds, assessing healing progress
  • Provide education on proper home health care procedures and strategies
  • Coordinate with healthcare teams to ensure comprehensive patient care
  • Keep patients comfortable through palliative and ambulatory care
  • Recommend tools or devices to improve patient quality of life

Requirements:

  • Current LPN licensure
  • Valid driver's license and reliable transportation
  • Proven dependability and reliability
  • Strong interpersonal skills for building patient rapport and trust
  • Initiative for independent decision-making and accepting direction
  • Ability to act swiftly in emergencies and work efficiently
  • Flexibility and patience for working in various living environments
  • Clinical skills, including wound care, medication administration, and vital sign monitoring
