Responsibilities- Conduct comprehensive evaluations to assess social, emotional, and support needs of patients and their families
- Develop measurable objectives in conjunction with the rehab team
- Collaborate with physicians and other health team members to understand social and emotional factors related to patient health problems
- Provide instruction and counseling to patients and families on treating and coping with social and emotional responses to illnesses
- Participate in care coordination activities and serve as a resource to other health team members in identifying and resolving patient needs
- Identify and assist patients and families in utilizing community resources to achieve measurable objectives
- Master's Degree in Clinical Social Work (LCSW)
- Current Licensed Clinical Social Worker licensure in the state of practice
- Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis
- CPR certification required
- Ability to lift 50 pounds and perform physical demands of the job
