As a key member of our interdisciplinary team, you will provide counseling services to support individuals with physical, mental, cognitive, and emotional adjustment through recovery.

Responsibilities
  • Conduct comprehensive evaluations to assess social, emotional, and support needs of patients and their families
  • Develop measurable objectives in conjunction with the rehab team
  • Collaborate with physicians and other health team members to understand social and emotional factors related to patient health problems
  • Provide instruction and counseling to patients and families on treating and coping with social and emotional responses to illnesses
  • Participate in care coordination activities and serve as a resource to other health team members in identifying and resolving patient needs
  • Identify and assist patients and families in utilizing community resources to achieve measurable objectives
Requirements
  • Master's Degree in Clinical Social Work (LCSW)
  • Current Licensed Clinical Social Worker licensure in the state of practice
  • Minimum of one year experience in a rehabilitation setting serving individuals with traumatic brain injury, spinal cord injury, or acute diagnosis
  • CPR certification required
  • Ability to lift 50 pounds and perform physical demands of the job
