Job Description

We are seeking a CoCM Behavioral Health Care Manager to join our team. As part of chronic care management, you will review patient charts and communicate with the Clinical department and providers about irregularities. This role is open to Registered Nurses (RNs) and candidates with a background in social work.

Main Responsibilities
  • Assessment and care management services:
    • Administration of validated rating scales.
    • Initiation of behavioral health care planning for behavioral or psychiatric health problems.
    • Revision and modification of care plans for patients not progressing or whose status changes.
    • Brief psychosocial interventions as needed.
  • Collaboration with the billing practitioner.
  • Maintenance of registry/tracking sheets.
  • Consultation with the psychiatric consultant.
  • Continuous relationship maintenance with patients.
  • Fostering integrated relationships with the care team.
  • Conducting interdisciplinary care plan meetings.
Qualifications
  • MSW or LCSW preferred.
  • RN (Registered Nurse).
  • Long-term care experience, preferred.
  • Behavioral health experience, preferred.
Compensation

$105,000 - $110,000 estimated salary, based on industry standards and location. This compensation package includes benefits such as health, dental, vision insurance, company-sponsored life insurance, short and long-term disability, and paid time off.
