Job Title: Community Health Worker for MSS

Sea Mar Community Health Centers is seeking a dedicated and compassionate Community Health Worker to join our Maternity Support Services (MSS) Program in Seattle, WA. As a key member of our team, you will play a vital role in promoting the health and well-being of new families in our community.

Job Summary:

The Community Health Worker will work closely with the MSS team to provide support and resources to pregnant and postpartum individuals, focusing on reducing negative birth outcomes and improving overall health outcomes. This role requires strong communication and interpersonal skills, as well as the ability to work independently and as part of a team.

Responsibilities:
  • Teach basic principles of self-care and promote appropriate utilization of pre- and post-natal care services.
  • Connect clients to available resources, including insurance enrollment, preventive health services, and community resources.
  • Provide referrals to healthcare providers and other resources to promote client well-being.
  • Enroll clients in the First Steps program and enter client information into our database.
  • Monitor and document clients' appointment attendance and engagement with services.
Qualifications:
  • High School diploma or equivalent required.
  • One year of experience in health and/or social services, preferably working with childbearing families.
  • Basic Life Support (BLS) CPR certification required within 90 days of hire.
  • Bilingual in English/Spanish preferred, but not required.
  • Reliable transportation, valid WA State Driver License, and auto insurance required.
