We are seeking a highly skilled and compassionate Patient Care Coordinator to join our team at ECHN.

Overview

As a Patient Care Coordinator, you will play a vital role in providing exceptional care to our patients. This is an exciting opportunity to work in a dynamic environment with a team of dedicated professionals who share your commitment to delivering high-quality patient care.

The successful candidate will have excellent communication skills, be able to work effectively in a fast-paced environment, and possess a strong understanding of patient needs and priorities.

Responsibilities
  • Assist patients with basic human needs and daily living activities under the guidance of registered nurses (RNs) or licensed practical nurses (LPNs)
  • Communicate effectively with patients, families, and other healthcare team members to ensure seamless care coordination
  • Perform additional competencies to enhance patient care under the direction of RNs or LPNs
  • Maintain accurate medical records and report any changes in patient status to RNs or LPNs
Requirements
  • High School graduate or equivalent
  • Successful completion of a certified nursing assistant program (CNA) preferred but not required
  • Active Certified Nurse Assistant certification upon hire
  • Basic Cardiac Life Support certification upon hire or within 30 days of employment
  • Excellent communication and interpersonal skills
  • Ability to read and communicate effectively in English
  • Basic computer skills
