About the Role:

This is a grant-funded position, contingent on funding availability. The successful candidate will work closely with healthcare providers, including OB-GYNs, hospitals, primary care physicians, and others, to ensure seamless care coordination. You will be responsible for conducting intake assessments, establishing care plans, and referring clients to relevant services and programs.

Responsibilities:

* Conduct intake assessments and establish care plans for clients.
* Focus on addressing social determinants of health, barriers to care, and high-risk pregnancy management.
* Provide high-risk pregnancy education, address appointment attendance, and medication adherence within your scope of services.
* Conduct client referrals to services and programs within the HHC and outside of the agency.
* Advocate for patients and help them overcome barriers to care, such as language barriers, transportation issues, or financial constraints.

Requirements:

* Associate's degree in Nursing, Health Sciences, Social Work, Health Promotion, or a related field.
* Two years of experience in maternal health with strong knowledge in high-risk pregnancy care coordination, public health, health promotion, or health sciences providing community education, direct healthcare services, or a closely related field to a diverse population.
* Valid driver's license required.

Benefits:

We offer a competitive salary of $62,000 per year, commensurate with experience. This is a full-time position, working 35 hours per week. Required to travel to site locations.
