Care Coordinator for High-Acuity Patients

We are seeking a skilled Care Coordinator to join our team at Charter Oak Health Ctr Inc. This is a challenging role that requires strong clinical knowledge, effective communication skills, and the ability to work independently.

Job Summary:

The Care Coordinator will be responsible for assessing and prioritizing patient needs, directing interventions for resolution. This includes evaluating patients quickly, providing care in one call, including scheduling, phone management, or directing to emergent care. The ideal candidate will have a strong understanding of clinical policies and guidelines, as well as excellent communication and interpersonal skills.

Key Responsibilities:
  • Telephone assessment and visualization of patient needs
  • Prioritizing and processing calls, deciding on urgent care, specialist referral, or primary care visit
  • Care planning for challenging patients
  • Ensuring patients receive appropriate care for symptoms or medical concerns
Requirements:
  • RN degree from an accredited school of nursing
  • Minimum 3 years of professional nursing experience in a clinical setting, with 1 year of acute care experience preferred
  • Minimum 1 year of supervisory experience preferred
  • Previous experience as a Triage RN is preferred
  • EMR experience, Epic knowledge are preferred
  • Proficiency in MS Office Word, Excel, PowerPoint, and Outlook required
Essential Position Duties:
  • Accessing patient information via EMR
  • Establishing therapeutic rapport with callers and resolving problems in one call
  • Reporting and documenting assessment information and patient changes in the EMR
  • Triage incoming phone calls, ensuring necessary communication and follow-up
  • Obtaining verbal orders from physicians, documenting and signing in the electronic system
  • Effectively prioritizing and processing calls for optimal patient care
  • Professionally identifying self to patients, family, caregivers, physicians, and other referral sources
  • Initiating and maintaining communication with the care team and VP of Nursing
  • Ensuring compliance with local, state, and federal laws, and established personnel policies and procedures
Compensation:

$80,000 - $110,000 per year, depending on experience.
