Overview:

The Cardio Metabolic Clinic (CMC) is seeking a highly skilled Registered Nurse to join our team as an RN Coordinator. This role will be responsible for providing professional nursing care and medical care prescribed by licensed healthcare providers. The successful candidate will assist in facilitating care coordination of patients to ensure positive outcomes and effective overall plans of care.

About the Role:

This is a full-time opportunity with an estimated salary range of $65,000 - $80,000 per year, depending on experience. The ideal candidate will have a minimum of three years of relevant nursing experience, preferably in clinic and cardiovascular nursing. A Bachelor of Science in Nursing (BSN) degree is preferred.

Responsibilities:

  • Collaborate with physicians from multi-specialties to develop referrals, coordinate clinic and outpatient visits, and navigate clinical care
  • Provide procedural coordination for applicable BH Program: LAAO, Structural Heart Program, etc., and other related procedures or diagnostic testing
  • Receives physician referrals and compiles clinical data for presentation to a multi-specialty physician team at scheduled program meetings
  • Perform clinical and social-economical assessment of referred patients and navigate care between physicians and diagnostic services
  • Educate patient and family about tests/procedures, results, medications, nutrition, and other health-related issues
  • Perform nursing care that requires professional knowledge and judgment for assigned providers, patient follow-up, patient teaching, and test result communication
  • Triage and respond to calls from patients, pharmacies, hospitals, nursing homes, and other medical offices in a timely manner

Requirements:

  • Graduation from an accredited Registered Nursing Program required; BSN preferred
  • Current Registered Nurse licensure from the State of Nebraska or approved compact state of residence as defined by the Nebraska Nurse Practice Act required
  • Minimum of three years of relevant nursing experience required; clinic and cardiovascular nursing experience preferred
